Almost everyone from the age of 45 should expect their visual performance to decline due to presbyopia. But that need not be definitive. This age-related and thus quite normal process cannot be stopped, but it can be treated. Up to now, classic reading or varifocal glasses, sometimes also intraocular lenses, contact lenses or the LASIK procedure have been used for treatment.
With the minimally invasive method PresbyMAX there is a gentle and qualitative alternative. Over 10,000 successful treatments have already been carried out worldwide.
Even with advanced presbyopia, it is possible to achieve excellent results. In this way, it is possible to do without reading glasses in the future and even correct other ametropias if necessary.
The PresbyMAX procedure is also an option for patients suffering from presbyopia and cataracts.
At EyeLaser, we only offer PresbyMax in combination with Femto-LASIK. This is a special technique that consists partly of monovision combined with multifocality.
1 . The preparation
We first disinfect your facial area. You will receive a local anaesthetic in the form of eye drops. The surgeon will gently fix your eyelids open using a special device. They will then ask you to look at the green light above you throughout the operation. All this is entirely symptom-free and painless.
2. The incision with the Femtosecond-Laser
The eye surgeon makes an incision in the cornea with a femtosecond laser to produce a 0.1 mm thin lid – the flap. The surgeon folds the flap back carefully. You may feel a slight pressure on the eye, and it may become dark for a short while.
3. The correction with the Excimer laser
Now, the eye surgeon uses the Excimer laser. With a computer-controlled laser, the exposed cornea is reshaped in a few seconds to compensate for the refractive error. The surgeon monitors the use of the laser through a microscope.
4. The completion
In the final step, the surgeon folds the flap back. It will adapt itself and form a protection for the cornea.
To protect your eye against environmental factors, we may fit temporary contact lenses. We will remove these the following day.
After the procedure, there is initially a significant change for the patient. Vision is perceived as different than with glasses. Patients usually (but not always) experience 3 phases (neuroadaptation):
All these phases together shouldn’t last for longer than six months.
A day after PresbyMAX surgery, you may feel abnormal sensations such as burning or stinging in the eye. The regeneration of the cornea with PresbyMAX surgery only takes 2-3 days.
There is the possibility that you will not accept the variation in vision between the two eyes. In this case, there is always the possibility of correcting the eyes for distance vision and then proceed with using reading glasses.
If your cornea is not suitable for laser eye treatment, you can have ICL surgery or refractive lens exchange with premium implants.

How your eye sees after PresbyMAX

The intermediate zone favours good vision at medium distances, e.g. when working at a computer. The distance zone ensures good vision at greater distances. The transition zone contributes to the stability of the treatment result.
The PresbyMAX laser eye treatment only takes a few minutes. The procedure does not touch or replace the delicate eye lens. The laser shapes the cornea using fast laser pulses.
The principle of the PresbyMAX method is like that of contact or intraocular lenses. A computer calculates the eye’s focal points exactly. The laser then shapes one cornea for near vision and the other eye for far vision. This results in good visual acuity at all distances. There are various treatment options for the PresbyMAX method. You will be able to read without your glasses within a few hours of treatment. Your distance and intermediate vision will improve significantly within a week. Complete, multifocal vision takes a few weeks before it is fully established.
